NON-REPAT ENTRY REQUESTS

  • EACH TRAVELER MUST APPLY ONLINE EVEN IF PART OF THE SAME TRAVELING GROUP

  • ALL REQUESTS MUST BE SUBMITTED ON OFFICIAL BUSINESS OR ASG DEPARTMENT LETTERHEAD

  • EACH APPLICANT ALSO REQUIRED TO REGISTER VIA THE DOH WEBSITE AT www.americansamoapublichealth.com.

  • All Applications must include and/or subject to the following:

    • Traveler must provide negative CV-19 test 72 hours before arrival

    • Traveler must disclose if they had a positive test result prior to the testing negative

    • ALL inbound travelers subject to 14 Day Quarantine if remaining overnight

    • ALL inbound ships (cargo & fishing) must provide the following 72 hours before arrival:

      • Number of cargo containers to be discharged/loading in PPG

      • Cargo Manifest (i.e. Containers from APW)

      • Port of Call/Voyage Memo for last 30 days

      • Official letter from Captain to report health status of all crew members to include daily recorded temperature within 5 days before arrival

      • Crew list (date of embarkation of each crew member)

      • Vessel particulars

      • Vaccination list (including Yellow Fever and MMR)

      • Maritime Declaration of Health

  • VISIT THE DEPARTMENT OF LEGAL AFFAIRS WEBSITE FOR THE MOST UP TO DATE REQUIREMENTS. ASG RESERVES THE RIGHT TO IMPOSE ADDITIONAL REQUIREMENTS NOT LISTED ON AN EMERGENCY DECLARATION AS THE SITUATION REQUIRES.
